Release and Waiver of Liability and Consent to Use Images

IN SIGNING THIS DOCUMENT, AND FOR GOOD AND VALUABLE CONSIDERATION, THE RECEIPT OF WHICH IS HEREBY ACKNOWLEDGED, I, THE RELEASOR, ACKNOWLEDGE THAT I UNDERSTAND THERE IS INHERENT RISK AND DANGER ASSOCIATED WITH ACTIVITIES INVOLVED WITH MY PARTICIPATION IN PROGRAMS WITH STUDIO SAT NAM. I ACCEPT FULL RESPONSIBILITY FOR THOSE RISKS AND DANGERS AND HEREBY WAIVE THE RIGHT TO SUE STUDIO SAT NAM INC. OR ANY DIRECTOR, OFFICER, EMPLOYEE OR AGENT ASSOCIATED WITH IT FOR MYSELF AND MY HEIRS, ASSIGNS AND PERSONAL REPRESENTATIVES.

I hereby release Studio Sat Nam Inc. and its directors, officers, employees and agents from any claims or liability howsoever caused from any and all actions, causes of action, damages, claims, demands or other liabilities, either at law or in equity, arising from or by reason of any bodily injury or personal injuries known or unknown, including death, which may occur as a result of my participation with Studio Sat Nam.  

Releasor hereby acknowledges that she/he has been fully advised of and has actual knowledge and conscious appreciation of the strenuous physical exertion involved in the activities in which the Releasor willingly participates. Releasor acknowledges that the opinions, services, instructions and other communications offered to me by Studio Sat Nam are not medical advice, consultation, therapy or treatment nor do they include any medical, mental, physical or spiritual diagnosis, whatsoever, nor are they intended to be in place of or in lieu of any medical advice or treatment that I may require, for any cause, now or in the future.

Releasor hereby authorizes Studio Sat Nam to use photographs or video of Releasor in its marketing materials and shall be permitted to use those images in its promotional marketing material without further authorization.

Releasor further acknowledges that Studio Sat Nam may not have liability insurance or any other insurance or assets available to cover any damages that Releasor may sustain.

Releasor realizes that s/he may be responsible for any and all injury to persons or damage to property which may occur as a result of or in connection with or as a result of her/his participation in these activities.

Releasor confirms that he/she is in good health and has no pre-existing medical or health condition which would or may prevent the Releasor from safely participating in the variety of activities or events held by Studio Sat Nam. The Releasor understands and agrees Studio Sat Nam accepts no responsibility for monitoring, care or treatment of any pre-existing medical or health condition while participating in any of Studio Sat Nam activities or events.

Releasor hereby expressly agrees that this Agreement is intended to be as broad and inclusive as permitted by law and that if any portion is held invalid, the balance shall, notwithstanding, continue in full force and effect.

Releasor hereby represents and warrants that she/he has carefully read this Release and Waiver, understands all of its contents, and executes it voluntarily and with full knowledge of its significance and confirms Studio Sat Nam has recommended the Releasor to obtain Independent Legal Advice on the effect of this Release and Waiver of Liability.
